Orientation Packet
Students who have been approved to complete their clinical experience requirements at Mayo Clinic Health System are required to complete our online orientation process. The following materials must be completed by the student:
- The orientation power point and certificate of completion
- A criminal background check
- Immunization records
- Health insurance information.
This must all be saved in the student’s file located in their home academic institution. All documents must be readily available for audit purposes.
The materials below must be completed by the student:
Orientation Presentation: Once completed fill out the Completion Agreement located at the conclusion of the presentation.
Wisconsin Caregiver Background Check: Students must have a Wisconsin caregiver background check dated within one year prior to rotation start (Contact your academic institution to confirm). This must also be housed within your student file. Any discrepancies MUST be cleared by a Mayo Clinic Health System Human Resources or Compliance/Legal employee.
The following immunizations must be completed prior to the first date of rotation:
- One tuberculin skin test within 12 months of start date, followed by a second-step TB skin test within 30 days of start date, or a negative Quantiferon blood test within 10 days of start date, or documentation as a previous reactor with chest x-ray within 6 months of start date
- Proof of measles, mumps, and rubella immunity by positive antibody titers or 2 doses of MMR
- Varicella immunity, by positive antibody titer of chickenpox or proof of 2 doses of varicella immunization
- Proof of Hepatitis B immunization or completion of a certification of declination of vaccine, if patient contact is anticipated
- Proof of tetanus/diphtheria immunization within 10 years
- Proof of influenza vaccination for students who will be on rotation at Mayo Clinic between October 1 and March 31
