Community Health Needs Assessment
A community health needs assessment, or CHNA, is a state, tribal, local or territorial health assessment that identifies key health needs and issues through systematic, comprehensive data collection and analysis, according to the CDC.
In the Southwest Wisconsin region, including the Sparta, Wisconsin area, we work with hundreds of community partners on collective efforts to strengthen the quality of life and improve the health and well-being of all community members.
These principles guide our efforts:
- Health is valued by all community members.
- Health is determined by many factors, including social, behavioral, environmental, as well as medical, and all are important.
- We are committed to working with others to produce measurable improvements in the health of all community members.
- Strategies to improve community health are best determined and implemented through partnerships between health care providers, community members and community-based organizations.
Community Health Needs Assessments identify priority issues that shape community engagement work.
Community engagement work in the Southwest Wisconsin region aligns with these priorities:
- Mental health and substance abuse
- Chronic disease and contributing factors
- Access to care
Healthy People 2030
We are pleased to be recognized as a Healthy People 2030 Champion by the Office of Disease Prevention and Health Promotion (ODPHP) within the U.S. Department of Health and Human Services. As a champion, we've demonstrated experience with disease prevention, health promotion, social determinants of health, health literacy, health disparities, health equity or well-being. Our mission also is aligned with the Healthy People 2030 objectives.
