Referral Operations Center
The Referral Operations Center (ROC) is your central point of contact for referring patients to Mayo Clinic Health System — Franciscan Healthcare in La Crosse.

To refer a patient, most clinical departments require a completed referral form, as well as the patient's most recent progress note. Fax this information to the number above, and Referral Operations Center staff will make contact with necessary departments. Some departments (Behavioral Health, Optometry and Primary Care) do not require referrals or these forms.
Referral Operations Center coordinators will facilitate your referral by ensuring all necessary information for the referral is acquired, and then routing the request and records to the appropriate department for review. When the appointment has been scheduled, they will notify you. When the appointment is complete, the department that has seen your patient will send you records from that appointment/encounter within three business days.
If your patient needs a referral on an emergent or "today" basis, please call the appropriate department directly, identify yourself (or your practice), and request to speak to a registered nurse or available provider. Be sure to indicate on the referral form (appointment timeline section) that the situation is urgent.
Referring a patient to multiple departments
Submit a referral form for each department where the patient will be seen that includes relevant information for that department. This will ensure that each department will have the correct diagnosis and information for the patient.
Scheduling your patient
When the completed referral form is received, Referral Operations Center staff will route the request and patient information to the appropriate department within 24 hours. The patient should hear from a scheduler within three business days.
For urgent referrals, the patient will be notified either the same or next day. Be sure to indicate on the referral form (appointment timeline section) that the situation is urgent.
Track patient progress via CareLink
All Mayo Clinic locations now use Epic for an electronic medical record platform. You can follow the progress of your patients when they are being seen at a Mayo Clinic Health System location using the Epic CareLink application. Visit the referring provider portal to access CareLink. First-time users will need to create an account. After an account is requested, a member of Information Technology will contact you and assist you with establishing a CareLink connection.
