Typical patients

Transitional care is designed to meet the needs of each patient who does not require comprehensive in-patient hospital care, but who is not quite ready to live independently at home. The type of patients utilizing the program typically include:

  • Orthopedic patients (knee and hip replacement surgery)
  • Cardiac patients
  • Stroke patients
  • Those receiving long-term IV antibiotics
  • Those undergoing wound care

The focus is to provide individualized therapy to best meet patient needs.
