Mayo Transitional Care following a hospital stay is often covered by Medicare and many private insurance plans. Social Services will review coverage with the patient and their family prior to transferring to Mayo Transitional Care and will monitor continued eligibility throughout the stay.
Medicare patients may qualify for the program if they fall into a category called "skilled care." The patient must require daily care provided by a nurse or therapist, not just routine care and must be making progress in their therapy or with their nursing needs. The patient must also have had a three-day regular hospital inpatient stay within 30 days of admission to Mayo Transitional Care.
