Social Services

Transitioning to or from the hospital can be difficult. Medical social services are available to assist all medical center patients in meeting immediate and/or post-hospital needs. Social Services offers the following services:

  • Crisis counseling, intervention and support
  • Community resource education and referrals
  • Hospital discharge planning referrals
    -Public health services (infants to geriatrics)
    -Personal care/supportive home care services
    -Skilled home care intervention
    -Hospice program
    -Assisted-living facilities
    -Skilled nursing/rehabilitation facilities
    -Home-delivered meals ("Meals on Wheels")
    -Transportation needs
    -Lifeline program
    -Durable medical equipment needs (walkers, wheelchairs, etc.)
  • Advance Directive information and counseling
  • Financial resource education and referrals
  • Protective services (infants to geriatrics)
